Free Dental Screening

Students may participate in a free dental screening by a licensed dental hygienist from Early Smiles Sacramento.

Participants will receive free dental supplies!

 Consent Forms can be downloaded or picked up in Room 1 from Mrs. Weinreich. 

Signed consent forms must be submitted to:

Mrs. Weinreich by December 20, 2023, to participate.

Turn in consent forms in Room 1 or email to Rose.Weinreich@sanjuan.edu

Forms are available in Spanish, Arabic, Farsi and Russian.
